The Social Worker is responsible for performing assessments, psychosocial services and interventions with the patient and family within their scope of practice as a member of the interdisciplinary team. This position requires the exercise of discretion and judgment to evaluate patients and families to determine needs and develop care strategies. The Social Services Specialist performs duties that include assessment, intervention, anticipatory grief counseling, care planning, end of life planning, documentation, collaborative/communication and the efficient use of various resources to hospice patients and families.     Qualifications   MSW from an approved college or university accredited by the Council on Social Work Ed
