Job SummaryMonitors and provides support to patients and facilitate various recovery-oriented activities.Job DutiesFacilitates or assists with adjunct groups/meetings and performs all associated duties, including documentation.Provides support to patients during evenings and weekends, and when needed, help to de-escalate any emotional reactions patients might have.Collects appropriate data on bio-psycho-socials.Completes initial family contacts and provide information on family services and document appropriately and facilitates sign-up for Family Day Program and monitor family visitations.Provides new patients with general orientation to the facility, including but not limited to, introducing new patient, initial assignment
