As a Behavioral Health Consult you need to know to:

Clinical Duties

  • Collaborate with Rocky Mountain Health Plans staff to prioritize hospitalized patients in need of intervention, track patient outcomes, and report program goals and metrics.

  • Collaborate with the Family Medicine Teaching Service attending faculty and chief resident to provide BH consultations to RMHP Medicaid patients, to include:

    • Provide BH consultations and co-visits during morning walking rounds.
    
    
  
  


• 
  
  
  
  • 
    
    
    
    • Attend seated rounds (as necessary) to create treatment and discharge plans with the collective FMTS team.

  • 
    
    
    
    • Assist in the coordination and co-facilitation of family conferences with patient,family/caregivers, and multidisciplinary team members.

    • Coordinate with the Psychiatric Evaluation Team and FMTS teams to determine the appropriate role for Inpatient Outpatient Behavioral Health Consultant in conducting mental health assessment and crisis intervention as needed.

    • Provide brief/focused mental health interventions and crisis management as needed during hospital admission.
    
    
  
  


• 
  
  
  
  • 
    
    
    
    • Provide substance use screening, referral and linkage to treatment for patients of all ages in psychiatric or substance abuse crises.

  • 
    
    
    
    • Coordinate care for post-discharge follow up and connection to appropriate mental health services.

    • Coordinate with hospital discharge planning team to ensure appropriate community referrals including referrals to shelter, housing, food resources, assisted living nursing homes, hospice, domestic violence resources, and Department of Human Services programs, including adult protective services reporting and child abuse reporting.

    • Serves as liaison for community mental health and substance abuse providers, law enforcement, DHS providers. Community involvement to improve systems/coordination of care.

    • Maintains a supply of resource materials to be distributed to patients as needed.
    
    
    • Provide in-home/community individual/couple/family therapy to include similar services as provided in the hospital. Ensures all work with individuals, staff, patients, and families adheres to confidentiality and HIPAA requirements.

  • Collaborate with physicians, BH Team, and other staff at the Family Medicine Center, offering co-visits, brief consultations, and other services helpful for assisting RMHP Medicaid patients to improve health, engage more successfully with primary care services, and avoid unnecessary hospitalization readmission.
  
  

Your experience.

• Master's degree in or doctorate degree in a mental health discipline that is licensable in Colorado, required


• Experience, sufficient for licensure


• Provisional Colorado license, with full license required within 3 months of hire, required


• Colorado Licensed Marriage and Family Therapist, Clinical Social Worker, Psychologist, or Professional Counselor, preferred
