The Social Services Assistant (Whole Health Partner) functions as an interdisciplinary team member, coordinating and connecting Veterans to Whole Health resources and programs. The position requires knowledge of internal and external resources to support the Veteran journey through the Whole Health System and the ability to connect Veterans to the best options to support their health and well-being while establishing an on-going relationship.
