PRINCIPAL DUTIES AND RESPONSIBILITIES:
Conducts individual therapy with assigned patients.
Conducts group therapy sessions..
Reports and updates pertinent patient information to physician and treatment team at team meetings.
Assesses patients mental and emotional status and informs others in a timely manner.
Identifies milieu deficits and assists in implementing appropriate measures to meet needs.
Develops coordinated treatment plan in collaboration with other members of the treatment team.
Track referrals of patients requesting services.
Ongoing coordination of care with medical providers and psychiatric providers inside and outside the ChristianaCare
Documents weekly ancillary notes in an accurate concise manner reflecting proper use of technology.
Participates in all milieu meetings, i.e., practice huddles, team meeting, clinical supervision, community meetings etc.
