JOB SUMMARY The Behavioral Health Specialist is responsible for coordinating and supporting mental health care within the clinic and coordinating referrals to other services outside the clinic. May provide evidence based treatments or work with other mental health providers when such treatment required. ESSENTIAL FUNCTIONS OF THE ROLE Supports and coordinates mental health care with the patient's primary care provider (PCP) and, when appropriate, other mental health providers. Serves as the lead contact to screen and assess patients in the primary care clinic for common mental health and substance abuse disorders. Provides education for patients with mental health and substance abuse disorders. Monitors patient in person, by telephone and virtually, for changes in clinical symptoms and treatment side effects or complications. Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications and effectiveness of treatment. Provides brief interventions using evidence based techniques such as Behavioral Activation, Problem Solving Treatment, Motivational Interviewing, or other treatments appropriate for primary care settings. Participates in regularly scheduled caseload consultation with the psychiatric consultant, and communicates resulting treatment recommendations to the patient's PCP. Consultations will focus on new patients or those that are not improving as expected. Communicates patient needs and treatment recommendations regularly through participation in PCP team huddles. Facilitates patient engagement and follow-up in care. Documents and tracks patient progress, treatment recommendations, follow up and clinical outcomes using a registry. Facilitates treatment plan changes for patients who are not improving as expected in consultation with the PCP and the psychiatric consultant. Utilizes a person in environment approach and facilitates appropriate referrals outside of the clinic. Completes relapse prevention plan with patients who are in remission. KEY SUCCESS FACTORS Ability to collaborate effectively in a team setting. Ability to maintain effective and professional relationships with patient and other members of the care team. Strong communication skills. Ability to effectively engage patients in a therapeutic relationship. Ability to work with patients by telephone or in person. Experience screening for common mental health and substance abuse disorders. Experience with assessment, to include utilizing standardized instruments, and treatment planning for common mental health disorders. Working knowledge of differential diagnosis of common mental health and/or chronic diseases. Working knowledge of evidence based psychosocial treatments for common mental health disorders. Proficient in teaching and applying brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation) Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role.
