You can work with a collaborative team of health professionals to improve a diverse group of patient outcomes and their quality of life while being patient focused and outcome driven. Our Lymphedema Certified Occupational Therapists plan, organize, coordinate and control a caseload of Home Care patients (homes and assisted living) in addition to those patients requiring specialized edema therapies. While acting as a consultant to clinicians and providing education regarding edema and edema therapy.
This OT position wouldsupport our North Metro region. Territory for the position include; Milaca, olgive, brooklyn center, maple grove, osseo , and surrounding areas. Home Base will be Princeton, MN
Day shifts, Monday-Friday
.8 FTE ( 64 hours in two weeks) and Fully Benefit eligible (PTO, retirement monies, wellness dollars)

Plans, organizes, coordinates and controls a caseload of home care patients, in addition to those patients requiring specialized edema therapy. Acts as a consultant to clinicians and provides education regarding edema and edema therapy.
Expectations include but not limited to:
Assesses evaluates the patients edema and establishes an individualized Complete Decongestive Therapy (CDT)
Continually evaluates whether the patient's 1) clinical needs can be met at home 2) are able to care for themselves or have a paid or voluntary primary caregiver 3) home environment supports home care services
Continually reassess resource utilization as patient progresses towards goals. Reviews, prioritize and implements POC based on payer limitations
Communicates consistently and effectively with patient, caregiver, significant others, physicians and all team members (Clinical resource, Utilization Reviews and MA specialists) to assure appropriate implementation and completions of the patient plan of care
Supervises paraprofessional staff in patient care. May delegate portions of CDS to trained professionals and paraprofessionals, including home health aides or cota.
Attends and participates in patient care rounds
Completes documentation on the day of service
Manages the patient care episode within payer limits (visits and supplies)
Provides counseling, teaching of patient, family and caregiver regarding care needs and health promotion/prevention
Stays current in home care regulations and all applicable licensing
Participates in educational programs, meetings and agency committees as appropriate
Participates in planning and presenting edema therapy in-services, orientations and staff competencies
Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served
Partners with patient care giver in care/decision making.
Communicates in a respective manner.
Ensures a safe, secure environment.
Individualizes plan of care to meet patient needs.
Modifies clinical interventions based on population served.
Provides patient education based on as assessment of learning needs of patient/care giver.
Fulfills all organizational requirements
Completes all required learning relevant to the role
Complies with all relevant laws, regulation and policies
Performs other duties as assigned.
1 year recent clinical (work) experience as an OTR in a rehab or acute care setting
Ability to function in job role independently
Licensed as an Occupational Therapist and eligible to practice in Minnesota
Certified Lymphedema Therapist - certification in Complete Decongestive Therapy (CDT)
Current CPR certification (BLS for Healthcare Providers)

Valid Minnesota drivers license (or license that matches your current permanent residence)
Current and adequate automobile insurance policy
Computer skills: able to document effectively in EMR
