Conducts initial patient interviews, gathering background, financial and other demographic data to be utilized in making eligibility determinations, referrals to specific programs within HOH and/or referrals to other agencies, programs or resources
Provides emotional support and psychosocial counseling to patients.
Collects factual information from the patient, family, community agencies and others to be used by treatment team staff.
Coordinates with staff in the development, implementation and evaluation of the patient’s individual plan of service.Provides patient’s family with information regarding patients progress and treatment options.
Screens patients for community home placement.
Monitors services delivered to patient to ensure that individual needs are meet.
May identify discharge needs and arrange for comprehensive services for patients following discharge from the hospital.
Maintains records and prepares reports related to the work.
Maintains patient information in confidential manner.
Attends hospital orientation, staff meetings and keeps all yearly mandatories up-to-date as required by HOH.
Perform all other duties as assigned.

Customer and Personal Service – Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services and evaluation of customer satisfaction.
Master's level degree in social work (MSW) or professional counselor required along with state license, LMSW or LLMSW.
Previous related field work experience preferred.
Excellent organizational and written/oral communication skills.
Must possess the ability to use discretion in confidential situations.
Detail oriented with the ability to handle multiple tasks simultaneously.
Ability to work independently and without constant direction.
Ability to work within a team environment.
Be able to maintain a professional appearance and attitude.
